"A normal battery is designed to retain up to 80% of its original capacity at 500 complete charge cycles when operating under normal conditions. The one-year warranty includes service coverage for a defective battery. If it is out of warranty, Apple offers battery service for a charge." In other words, if it is below 80% maximum capacity, it is below normal performance. You can still use it but you may wish to consider having it changed.

Depends on your location. In many countries, Apple doesn't directly provide the service and the authorized repair shop can set its own price. Even worse is that some of these shops may charge a mandatory "diagnostic fee" just to look at it even if they determine that it's not eligible for a battery replacement (such as damage that precludes the service or that the battery isn't depleted enough to be eligible for a replacement from an authorized shop).
