Is there a monthly service charge to be able to take phone calls on the Apple Watch?

The cellular watch needs an eSIM provision from your iPhone cellular provider in order to make calls whilst away from the iPhone, and this will incur an extra charge. In USA it is about $10 a month.

You can make calls without it on the Watch if the iPhone is within bluetooth range.

Family paired cellular watches (paired to a parent's iPhone) need their own separate plan and can be from another provider, but yes nothing is free.
