We aren’t aware of a known issue with Emergency Bypass, but it would be our pleasure to assist today.
When you have Emergency Bypass enabled for a contact, it should allow their calls and/or messages to come through even when you have the ring switch set to silent or a Focus enabled. If you’re not receiving calls or messages from your spouse or sibling with this feature enabled, we can see why you’re concerned.
It sounds like you’ve already done quite a bit of troubleshooting, and we wanted to clarify a few things to ensure we’re on the same page because we’ve found that some terms can mean different steps to different people.
• When you say “hard restart” what exact steps did you take?
• What do you mean when you say “validated” your Focus settings?
We’d also like to ensure your notifications are working correctly in general. If you fully restart your iPhone and set the ring switch to Ring mode, with all Focuses disabled, do you hear incoming notifications from them? These articles will help you with each step.

Use Ring mode
To put your iPhone in Ring mode, move the switch so that orange isn't showing.

Turn off a Focus
When you’re finished using a Focus, you can quickly turn it off to allow notifications again. After you turn off a Focus, it still appears in Control Center and can be reused.
1. Do any of the following:
* Touch and hold the Focus icon on the Lock Screen.
* Open Control Center, then tap Focus.
4. Tap the Focus that’s on to turn it off.
