The region of your iTunes Store always matches the region of the Apple ID account. Each Store region has their own catalog, where titles have language support aimed to match that region. While possibly inconvenient for frequent international travelers, that is how content is currently licensed to Stores.

Which languages are included for any title in any region, is determined by the supplying studio. That may not always include English. (Some countries seem to have a general preference for dubbed content, or so the studios think.)

You can view what languages are supported for any title in your region, by looking for the listed languages on the Details page for any title in the Apple TV app. When multiple languages are supported for a title, you may change from the default as described here: Change subtitles or languages for videos in the Apple TV app or iTunes Store - Apple Support.
