After update iOS to iOS 16.2 focus modes don’t work properly, I deleted all, add new and still not working. My focus modes on and off based on my location. Sometimes they work, but often not. I have to change it manually.

It sounds like you're having trouble using the Focus feature on your iPhone. We can help.
Go ahead and try these steps next to Use Focus on your iPhone or iPad. If you are using location as a trigger, it may be worth checking to see if your location services are working how you expect by testing with Maps or Find My.
If you still aren't seeing any improvement, you can Contact Apple for support and service to report the behavior.

Maps and find my work perfect, only focus modes don't. I have two or three locations in one Focus mode to trigger on, and only works in one.
