All I did was charge the phone to go set it at my phone provider and it's asking for a 4 digit passcode. It's already security locked from failed attempts to unlock it. How do I open my phone without it being set up ?
I haven't set anything up yet. Can't remember the passcode I chose I've been in the hospital for 3 months. It's not giving an option to erase or factory reset and it was not connected to anything yet. No downloads etc haven't even put the sim card in.

You won't be "opening" the phone, you will be resetting / erasing it so that you can set it up again using the instructions in the support document linked above your name.
If you have a current backup of the phone, you can use that to restore the phone.
