Settings>Cellular>Personal Hotspot>OFF
You are saying this setting changes itself to ON (?)
Do you ever use the Hotspot intentionally?
You can try changing the existing hotspot password on the iPhone.
On the Mac go to Keychain Access.app an search for your hotspot name and delete the entry
