Why do I keep getting in-app game charges that I did not make?

I play one game on my iPhone where I make in-app purchases. I require a password for every purchase to prevent accidental charges. However I keep getting duplicate charges that I need to request refunds for. And today, I got two charges that I did not make at all. The Report A Problem function only allows me to select "I didn't mean to buy this" and not "I know I didn't buy this." No one else has access to my phone or knows my password. Suggestions please?
