Any advice to be able to scan documents using notes app on iphone 12 running 14.6 IOS? I don't have the camera icon when creating new or editing existing notes and app says Notes App Needs Upgraded to Create directly from home screen (using firm press and hold method). How do I upgrade notes app? Phone and app are both up-to-date.

Make sure the note was created under the iCloud or "On My iPhone" groups. Other accounts may not support the extra Notes options like Document scanning.
As to the Upgrade notice, go to Settings->Account (top of settings)->iCloud->Notes, and make sure its turned on.
