Does blood oxygen and ecg work on a family member watch
the family member paired watch doesn’t show the ECG and blood oxygen apps. Why not?

No, as explained in the very support document you posted from, those features are not available with Family Setup, since there is no iPhone to store them in and can get mixed up with the owner of the iPhone used to set them up if they were to be stored there.
Some concessions need to be made with Family Setup. If you want all the features, then the watch needs to be paired to its own iPhone.
