I bought a subscription for an app that is no longer available at the App Store, how do I access the app that I paid for

You don’t. If the developer of that app pulled it or if Apple pulled it for violating App Store terms then it’s gone. You should contact the developer of the app to find out its status and if they plan to return it to the App Store.
