THIS PROBLEM IS ONLY ON APPLE TV 4K. IF I GO THROUGH THE XBOX ONE IT'S JUST FINE. IF I GO THROUGH HULU AND NETFLIX ON THE TV ITSELF THE PROBLEMS DON'T OCCUR.
I NEED HELP WITH THIS. It makes the AppleTv 4K useless.

I'm having the same issue. It's just very recent, so I think it has to do with the 14.7 update. I've tested it similarly to the original poster. It is NOT App related. All audio is out of sync, not just a particular app, it's the interface too. Every little beep of the home screen navigation is delayed. So far, I have tried:
The only thing that seems to work is to unplug it from both the power and HDMI and just let it sit awhile. Then it's back in sync for maybe half a day to a day and then it's time to repeat the exercise again.
It has to be an AppleTV software issue.

Well I found a fix that might help the rest of you. If you go back to the main menu of Apple TV and double click the small TV icon to the right of your menu button on the remote. It shows all the applications that are currently in the buffer just like your phone or an iPad. Throw away current app.
Interesting that Apple would blame the app as miss behaving when it happens on their own app. There must be some fundamental difference in apps for 4K them for all the other Apple TV‘s that I own. Sounds like a hardware problem
