Why does my iPhone not have the option volte option? so I can’t merge callls when making important phone calls with doctor hospital and mental health team but my partner has the same phone but his has the option ?

Support for volte is a carrier feature. If your carrier doesn’t support it, then it is not an option for you. So if you’re supposed to have it and don’t, contact your carrier for support about why that feature isn’t appearing in your device.
