The ECG app doesn’t show on my watch. I don’t see it in Apple store either. Can someone help me?

As you didn't say in your original post that you'd checked that, it was necessary to ask.
Go to Settings>General>Language & Region>Region and, regardless of what it says, re-select your region as United States.

What region is your Apple ID connected to? Where did you purchase your Apple Watch? Was it intended for sale in that country?
