I have just update my apple watch 3. And the update stated that the function is available in South Africa. But when i wanted to set up the notification it tells me it is not available in my region. Is it only for apple 4 watches

What country did you buy the watch from?;
For the ECG app to work on your watch the ECG app has to be approved by;
A. the government of the country in which the watch was bought and
B.the government of the country in which you reside.
