watchOS Feature Availability - https://www.apple.com/watchos/feature-availability/#branded-ecg

Taking an ECG with the ECG app on Apple Watch Series 4 or later - https://support.apple.com/HT208955 - "Make sure that the ECG app is available in your country or region and that you're in the country or region where your device was purchased." So unless it is supported both in the country of purchase and where you have it, it won't work.

In general Apple strongly recommends against purchasing mobile devices for use in another country.

Apple Watch - Carriers - Apple - https://www.apple.com/watch/cellular/ "To cover the LTE and UMTS bands used around the world, there are two separate models for Apple Watch Series 5, and three for Apple Watch Series 3.2 They have been optimized for use in the country of purchase, and not all models work in all countries or regions. There is no single model that supports roaming worldwide. To see which carriers in your country or region offer cellular service for Apple Watch, refer to the chart

LTE Find the right iPhone for the right country or region. - http://www.apple.com/iphone/LTE/

Warranty restriction! - http://www.apple.com/legal/warranty/products/ios-warranty-document-us.html - "IMPORTANT RESTRICTION FOR iPHONE, iPAD AND APPLE TV SERVICE. Apple may restrict warranty service for iPhone, iPad and Apple TV to the country where Apple or its Authorized Distributors originally sold the device."

Because ECG is a medical device, Apple does not want to have it product banned because it allowed ECG use in an unapproved manor.

Unless Apple got some certified proof that you were a citizen of a country that approved ECG, then it has to guess. It knows where it shipped the Apple Watch for sale and whether that country has approved the app, with the assumption that it is the citizens of that country that purchased the Apple Watch. It knows based on IP addresses where the Apple Watch is being used and if that country has approved ECG.

What it does not know is if the person using the Apple Watch is a citizen from a country that has approved ECG. So it is doing the best it can to not cause governments to ban the sale of Apple Watches because they allowed their citizens to use an unapproved medical product.
