Hello,
I recently bought Apple Watch 5 in Israel and now I am located in Czech Republic.
ECG feature is supported in Czech Republic as documented on Apple support page.
I was sure that once I will be in a supported country the ECG feature will become available.
But that didn’t happen.
Tried to unpair and repairing as new watch but that didn’t work.
Any advice?

Taking an ECG with the ECG app on Apple Watch Series 4 or later - https://support.apple.com/HT208955 - "Make sure that the ECG app is available in your country or region and that you're in the country or region where your device was purchased."
So unless it is supported both in the country of purchase and where you have it, it won't work.
