When you pay the $2.99 a month for 200gb of iCloud storage do you get 200gb each month ?

You don’t accumulate an additional 200Gb every month. You get a fixed amount of storage, 200Gb and you pay monthly to maintain access to that.

Yes, consider it just like renting the space on iCloud. It is a monthly rental fee.
