My wife has been using Voiceover for years so she is familial with it. The problem is that the Voiceover voice will speak over the Siri voice was when VO is turned on. We've tried a lot of settings and nothing has changed. She's blind and not tech savvy so this is very frustrating to her. I've noticed others that just upgraded to 18.6 are having the same problem. I just upgraded my Iphone 16 to IOS 26.1 and it wiped out all my notes plus messed up my contact list. I have never been impressed with the Apple Updates. I always dread seeing a new release is available and eventually being forced to upgrade. Not being hateful just factual.
