You don't give permission for Apple apps to access other Apple apps. Permissions are only used for third party apps. That is why the Phone app will show a name of a person in your Contacts instead of just the phone number. You also cannot stop that. The Mail app will show the name of the person instead of the email address and Messages will show the name of the person in your Contacts.

What app were you using for the conversation? Messages, WhatsApp, Messenger, SnapChat or something else?

Go to Settings —> Apps —> Health and disable Contacts access from there.

There is no way to prevent one Apple App from accessing other Apple apps. However, none of that information leaves your phone so there is no privacy risk.
