I am trying to prevent my phone from access to private browsing and downloading on the App Store. The normal method of “limit sites” and “don’t allow downloads” suddenly stopped working.
what do I need to do to get those restrictions running again?

If you want to block Private Browsing and stop App Store downloads, you’ll need to do it through Screen Time. Go to Settings > Screen Time > Content & Privacy Restrictions and turn that on.
If it’s for a kid’s phone, you’ll need to set it up with Family Sharing so they can’t just switch it off.

So, the odd thing is that ALL those steps are done. And yet the App Store still remains accessible and private browsing isn’t turned off.

What’s that green-icon app? QuickBooks Workforce? Any IT-related or IT-managed apps installed?
If this is a business-related Mac as it appears, I’d head toward establishing a managed or supervised Mac, as access to the App Store is a requirement for app and system updates.
It Opens like normal unfortunately! Same issue with Private browsing. And I restarted the phone last night to try to jog it. No change. Thing is, a couple months ago it worked like normal.
now I could try unlocking the device. Turning the stuff from off to on, restarting the device, then from on to off?

You cannot block the App Store. Not entirely. A profile restricting access to the App Store can help.
Private Browsing can be restricted with a Restriction profile 'Web Content Filter' using 'Automatic Filtering', as well.
Basically, manage or supervise this iPhone.
I don't believe you can disable private browsing.
