My 15 pro and my husband, daughter and mom are the people who call the most. Their calls or FaceTimes only ring once. It still shows on display.

Ok, reach out to your carrier/Apple Support then. I can’t think of anything else which might cause this specific behaviour.
Sounds like Attention Aware is kicking in. This senses when you are looking at the phone and assumes you see the notification then it silences the ringer.
Go to: Settings > Accessibility > Face ID & Attention, and turn off Attention Aware Features. Now check again and let us know what you find. Thanks!
