I have Ventura 13.6.1 - my Photos are working really well and albums are just perfect.
If I download ventura 13.6.7 - will the Photos program change? Any difference in layout and format?
If I am happy with 13.6.1 do I need to upgrade?

Within a major OS, there are not big changes. Some bug fixes and security updates, mostly. But really, nothing has changed with Albums, folders, and organization, even in Sequoia.
I would make a copy of the Photos Library (in addition to Time Machine,) because that's always a good thing to do.
Really, though-- go for Sequoia!

Well, I figured that might be it. And, you're lucky-- Ventura was a very stable system that had introduced some good features over Monterey. .
I had a 2017 27" iMac and had no problems with my Photos library due to the updates thru all of the point updates of Ventura.
You're asking about a point upgrade on a three year old OS? I don't recall any alterations but then I keep waling into rooms and not remembering why I did.
If you run older Operating Systems you're missing out on bug fixes and security updates. YOU decide if you need to update.
Here's what you do - and this works for every update and upgrade.
Back up. Back up again. Upgrade. If the upgrade causes you a problem then roll back with one of the back ups you made. Simple.
