Apple Music keeps multiplying songs in playlists - 256x the same song

I am subscribed to Apple Music and am experiencing an issue regarding playlists.

For some reason, the songs in my playlist keep multiplying. One of my playlists now contains 12160 songs, each song about 256 times.

I listen to these playlists regularly. The library is used across multiple devices (MacOS, iOS, Windows), with my iPhone being the only device where I sync for offline listening.

I only use Apple Music for streaming, I don't have any purchased songs in my library.

I know how to manually remove duplicates, however this keeps happening even after de-duplicating the lists and I am now at a point where I don't want to clean up 255 copies of the same song.

Is there anything I can do to fix this?
