Songs from same downloaded album showing as multiple albums in iOS Apple Music

This just started happening a week or so ago. Albums I've downloaded onto my iPhone start randomly showing some tracks as different albums. One day the album is fine; a day later it's split, and won't go back.

In the attached example, the album is called Mend, and those other two instance are the names of single songs.

The albums do not appear this way in iTunes - all have the same album info; Artist, Album Artist, Year, etc.

In this case I'm talking about downloaded albums, but this is possibly happening to non-downloaded ones too - I haven't checked yet.

This is very frustrating.
