Unable to add Covid QR code data to Apple Health
Trying to add either one of the QR codes results with ... Unable to Add Data This data could not be added to Helath.

Check with your State's Public Health department. Not all COVID information can be directly added to Apple Wallet. Some has to be done via your Public Health department. Ask them, or the place you received your vaccination from if they have information.
