How do I retrieve an iPhone backup and restore it on a different device?

Nope it is your responsibility to back up your device to icloud.

if applicable you would update the ios on your iphone 12, then erase and use the 15 backup if you have one.

As for the code use your other approved devices to get it.

Apple does not back up your phone at all. Either you use iCloud backups, which happen automatically when enabled when the device is plugged into power and connected to wifi over night, or you back it up to your computer on a regular basis.
