Apple password app creating same password for different accts

I changed my password on three different accounts using the password app to regenerate strong passwords. Later when looking in the app I found that all three passwords it generated were the same. So I went back and changed two of those again and the same thing happened, same password for different accounts which of course is bad. Any knowledge of why this is happening would help.
