disk not ejected properly Mac M1 running Sequoia 15.3.2

I have seen this before on other Apple systems I've owned over the years. The advice is always the same (as I've seen posted here): unplug, check cords, disks, blah blah blah. These are never the problem. It's an error in the OS which changes over the year have fixed then broken again. Could we just get it fixed once and for all?

Attempt to unmount


Finding the offending file and application


It's usually Spotlight


Sometimes Spotlight is so hung up, you can't add the volume to the Spotlight privacy list
