Refund request for unintended app purchase linked to my Apple Account

I ordered something an app purchased from a game and I got that but at the same time they added something onto it that I did not order and did not receive, but I had to pay for it. I asked for a refund and I didn’t get it. This has happened to me twice now I’ve had the same thing in the past and it got refunded but these last two times I haven’t done. It is from the same app. The app says that it’s a technical problem to ask Apple For a refund. That’s what I did and I got refused. I redid it to find out why and they won’t answer why and I wanna know why I can’t get my refund. I didn’t order it. I didn’t get it. Why should I pay for
