Black square appearing on photos taken on iPhone 16 Pro Max camera

When I take pictures using the camera on my iphone 16 pro max a black square appears on the picture and doesn’t t go away even when I try to edit the photo ios 18.3.1. Why is there a black square randomly appears on my pictures? The square seems to be a quiet independent entity cuz it doesn’t go away or respond and it places it self any where on the picture! I went to Apple store and they had the diagnostic test done and everything was good.
