You asked a question in the title of your post - How can I open a password-protected PDF on my iPad?

The answer is this - the only way to open a password protected PDF (anywhere!), you use enter the password. If you are not the one who set that password, then contact the person who set it up.

I could not quite understand what you meant by what you wrote after that but it seems you opened a form, then filled it out. Is it possible the provider only meant for you to print the filled out form? Or to do a Save As which would not affect the original document.
