Why are my notifications randomly very loud

My notifications (calendar reminders, email, app push, etc.) are randomly very loud, regardless of what the system sound volume is, except when the silent switch is activated. I have tried restarting the phone, double-checked settings, everything short of simply factory resetting the phone. Any ideas? iPhone 15 Plus, iOS 18.2
