ICloud pop up every 5 seconds

Using my Windows software on my computer, an ICloud pop up window keeps coming up asking me for my password. I put in my password; then two-factor authentication comes in; I allow on my IPhone and type in the 6-digit verification code. Everything appears to be okay; then the window pops up again. Can't get rid of it.
