My MDM has downloaded iPadOS 18.1.1 on a iPhone with iOS 17.4.1 (build 21E236). Is it going to be an issue since it's not an iPad ?

It's not going to install at all if that's the case. The installer verifies the device is compatible before installing./
Are you sure it's iPadOS?
Where are you seeing its iPadOS exactly?

In the MDM console, under OS Update Version : iPadOS 18.1.1
Contact your MDM vendor.
In that case, it won't Apply to your iPhone at all.
The MDM should have iOS 18.1.1 for iPhone in addition to iPadOS.
They are separate updates.
Even if you try to install it, it will probably fail as it's not a compatible device for it.
