Watch History and Up Next Fail

I've had my Apple TV (and Apple TV+) for over a year. I just did a factory reset on my Apple TV because it wasnt keeping track of watched episodes correctly. Especially on Apple TV+ shows and movies. I thought the factory reset solved the problem because I noticed almost immediately that everything was working perfectly. But after a couple days, Apple TV started having "amnesia" again and began loosing track of episodes (I know for a fact) I already watched (or partially watched). Is anyone else experiencing this ?
