Family Sharing - Why does my spouse need to ask to buy?

I have family sharing turned on and my wife listed as a parent/guardian. Yet she needs my permission before purchasing songs. I don’t see any way to force the settings to allow her to purchase at will.

Her Apple ID says she’s over 18 and it's an adult account
Her name does not appear in Ask To Buy
When I click on her account through Family, there is not "Ask to Buy" button as there are for the child accounts

This has been asked here several times before, but the stock answers that are provided don't actually address the question and don't provide an actual solution.

Maybe they haven't been answered since from everything I have ever seen or read this is essentially impossible to set up even if you wanted to? (And no, I don't remember others saying this is happened to them.) If you think this is happening then you need to talk to Apple.
