How to fix "White Square" error message when adding debit card online

Please help When I click on the plus to add my debit card, a white square appears and it tells me that I can’t add any card what to do?

Debit card shows “failed” right after adding on the App Store I was having an issue where I couldn’t download any (even free) apps and I had to add a payment option. I added my debit card but right after I added it, it got marked as failed. I know websites and such make a test run of the card (usually about $1 USD) so I checked my account and I have over a dollar. Is there anyway to fix this?
