When you’ve agreed to the terms and conditions, the approval process is entirely with the bank, payment network, and possibly a third party verification service. It could be an issue with tokens, or something with your bank, payment network, etc. when there is a conflict with information they have regarding verification of your device and your identity.
Generally, banks have an internal fraud identification process they use to approve cards for adding to a Wallet app. The fact it may have been added to one device does not mean there is not conflicting information on your iPhone. Simple matters like too many tokens can generally be fixed by the bank or payment network. Flags for fraud are more difficult to track down. It could be something as simple as multiple iTunes account, a wrong address, conflicting phone numbers, a name issue and others. Banks generally don’t reveal what information they have access to and how they verify the device is the credit card account owners.
Have tried entering the card more than 5 times today?
I had the same issue when I upgraded to a new iphone. I tried all sorts of resets, changing my region back and forth, disabling passcode and re-enabling it, contacted apple support, contacted bank support, etc. etc. I can't say any of that really worked, and I think constantly retrying over and over again probably made matters worse.
Ultimately, after a few weeks I could start adding some cards again, and after additional weeks I could add all of my cards as normal. Based on my personal experience, I recommend you leave apple pay alone for a few weeks and retry after several weeks have passed.

Do you receive the terms and conditions and agree to them? Or do you see the message before receiving the T&C’s?
Yes I saw that after agreed the T&C’s that’s the error message popped up inside the screenshot I sent you earlier
What did the issuing bank say when you called them?
After contacted my bank I was told that nothing wrong with my card that my debit card is good and my bank is working perfectly
