This is a user-to-user forum. No one here can connect you to anyone. You can start with the support link at the lower left of every page of these forums. There is usually an option to request that Apple Support call you.

What is the version system software? Find out which macOS your Mac is using - Apple Support

Please describe the issue. What happens instead of the expected? Do you get an error message? What does it say exactly?

Only your carrier can provide a PUK code. You'll need to contact them.
