How can I download or extract data from the Health App?

I'd take a look at the app "Heart Reports". It will allow you to pull reports quite easily. It's only $3.99 for the full version.
You can also extract all the data for free from Health but then you have to deal with a very large spreadsheet with all of your data.

The only export format from Health for all health records is XML. It is not exported as PDF. And yes, you can export all the records, not just the ones from your provider.

To add to my comment on idisseabright’s reply. The only records you can export are your medical records from designated health centers you use. The records coming from your watch don’t appear as an
option to download - at least in the latest version of the Health App and the latest version of IOS on the latest version of the IPhone
