I received an offer for 5 months free Apple TV. We watched one episode of a series last night but this evening it's asking me to sign up for a free week. There is no sign of my subscription at all? Can anyone please help?

Who offered you the 5 months free trial? Did you redeem as they described? It is not automatic, you have to do the redeem procedure, and link your Apple ID account.
Then make to sure to be signed in to the Apple TV app, through the in-app Settings (sprockets wheel icon), then Accounts, and enter your Apple ID account credentials there.
