Why do I have to have security access.

Setting up a Recovery Key is completely optional. I would not give anyone access to your iPhone or Apple ID.

I'd recommend setting up Medical ID - Set up your Medical ID in the Health app on your iPhone - Apple Support. This allows you to put important health information that is accessible to anyone, including first responders, while your iPhone is locked.

If you need to go the hospital and are unresponsive, they can access your Medical ID on iPhone or Apple Watch and see your:

1. Name
2. Age
3. Weight
4. Height
5. Primary Language
6. Medical Conditions
7. Medical Notes
8. Allergies & Reactions
9. Medications you're currently taking
10. Blood Type
11. Organ Donor Status
12. Emergency Contacts

All of this is visible while the device is locked! Meaning no one will have access to your other data.
