Content Restrictions keep turning on

I’m about to lose my mind. I have an iPhone 14, running the latest 16.4.1 OS. For the last 6 months or so, I would periodically unlock my phone to find that Content Restrictions had been enabled. This removes Safari and the App Store, and locks down Amazon and some other things.

Turning off the Content Restrictions brings all of that stuff back, but my apps have been completely rearranged and move to different screens and pulled out of folders and I can’t find anything.

Starting today, this is happening literally every two hours. I have given up rearranging my apps.

I have other devices that my kids use with Content restrictions, so I’m guessing this is somehow related, but the only kid device that is actually using my Apple ID is turned off with a dead battery and has been that way for at least a couple weeks. All of the other devices are using my kids’ Apple IDs. I even checked the list of devices related to my Apple ID and there is nothing on there that shouldn’t be.

I’m out of ideas.

Edit: possibly related, over the years my email address has changed. I have updated to my latest email everywhere possible, and that is the one that I used to login. But if I go into my account settings on my phone and try to add my most recent email address, it says that email address is invalid. And it is showing the oldest possible of all of my email addresses as my “primary”. Maybe I have a ghost account floating around somewhere?!?!
