If you haven’t already done so, perform a forced restart of your iPad - as this will often clear an isolated software issue:
Force restart iPad (models with Face ID or with Touch ID in the top button)
If your iPad doesn’t have the Home button, do the following:
Force restart iPad (models with the Home button)
Press and hold the top button and the Home button at the same time. When the Apple logo appears, release both buttons.
If the problem persists, please contact Apple Support - as they may need to escalate the issue. Apple Support can be contacted here:
Alternatively and perhaps easier, you can initiate and manage your support cases using the excellent Apple Support App. If not already installed on your iPad, the App can be downloaded from the App Store:

You would also be well advised to submit a Bug Report via Apple’s Product Feedback portal. For iPad and iPadOS:
When submitting your Bug Report, please ensure that you include details of your iPad model and the installed version of iPadOS within the body-text of the report. Please don’t rely upon the pick-list - as this often does not include the latest models or most recent release. Every submitted report counts, as until sufficient reports are received, Apple may not assign resources to investigate and correct the issue.

Where potential bugs are involved, even if you have an open case with Apple Support, it remains essential to submit reports via the Product Feedback portal.
As for the [A1619] Lightning to USB3 Camera Adapter, this may require a firmware update. Current firmware for this adapter is 1.0.5.
I have tested on multiple iPads and different generations. It is not a Software issue. I also have a different knock off adapter that has ethernet built-in and it appears to work.
Did you get beyond the second line of my reply?
Yes I also spoke with support and did it.
On latest firmware. I did find that it does work with the 20W USB-C block and USb-C -> Lightning cable. But we have had issues with those in the past where they stop working due to power delivery.
Actually tried it on another iPad and it did not work with the 20 Watt adapter.....Gotta love it
