Assuming that both your iPad and iPhone are signed-in to iCloud with the same AppleID - have you enable iCloud Notes on both your iPad and iPhone?
Settings > [Your Name / AppleID] > iCloud > [Apps Using iCloud] Show All > Notes > Sync this iPad/iPhone - set to ON
