i forgot my phone passcode and my apple id password ?

For your iPhone passcode you will have to restore the phone to factory settings, using one of the options below.

After restoring the phone using one of the methods above you can restore your most recent backup.

Once you have the password, you will need to reset / erase the iPhone and then set it up again. If you have a current backup, you can use that to restore the phone.
