Start with a forced reset to rule out confused software. Press and hold (together) the Digital Crown and the Side Button until the Apple Logo appears on screen. Release the buttons and let the watch reboot, enter the passcode when the restart is completed.

If you forgot your passcode you will need to unpair and erase the watch, then pair and setup again. Before you start doing this you should make sure you know the Apple ID for both the watch and the iPhone because you will need it for Activation Lock.
