I'm using a 2021 M1 MacBook Pro running macOS 13.2.1. Recently, upon restart & startup, the VoiceOver utility appears at Login screen. VoiceOver is turned off in System Settings but that makes no difference. Safe Mode is the same.
Any ideas?

You may need to change your Lock Screen settings instead to get VoiceOver to not appear where you want it.
Try following these steps to manage VoiceOver in the login window:
> System Settings, then click Lock Screen 
in the sidebar. (You may need to scroll down.)Automatically use VoiceOver in the login window on Mac
That should get VoiceOver turned off for you.

Thanks. I already tried that along with various other fixes including setting up a test user account on the Mac. Voiceover remains turned off yet it activates upon all restarts. I spoke with Apple and they reviewed it and asked me to do the same steps I'd already taken. I also did a screen share with them. They ended up saying they'd never see this issue and want me to take it to the Genius Bar. Good thing I have a spare Mac.
