My I-phone 12 - goes straight to vm and/or has missed calls all the time and the phone is usually right next to me and the ringer in on. Can you help me to understand why this is happening?

Does this only seem to happen when people call from numbers not in your contacts? If so, this may indicate that you have Silence Unknown Callers on in Settings > Phone.
If not, could you also check to see if you currently have a focus enabled? This can help with checking: Use Focus on your iPhone or iPad
Checking these things can help narrow down what may be happening.
